Welcome to the Child Care Survey Application

The Child Care Application Portal houses the GSA Annual Child Care Survey, the Quarterly Enrollment Report for each of the GSA Child Care Facilities, and the Parent Survey.

The GSA Annual Child Care Survey is a web-based application used to collect child care center information from the providers in GSA managed Child Care facilities as of January.
